Hazel Health

The District is proud to announce a new partnership with Hazel Health to bring high-quality, school-centered healthcare to all students in grades kindergarten through sixth. Whether a child needs care for a sore throat or help managing stress, Hazel Health makes it easy to connect with expert clinicians—right from school or home through virtual appointments. 

Hazel Health ensures that every child receives the care they deserve. With no out-of-pocket cost, language support, and access to licensed professionals, this program removes barriers to healthcare and supports the whole child.

To participate, families must complete a Hazel Health Consent Form, and return it to your school's front office. You may find the forms below to download and print. If you need assistance, please contact your school's parent coordinator or health technician for help. 

School staff, legal guardians, and Hazel physical health providers can refer students to therapy. If you are looking to refer your child to Hazel therapy, please call 1-800-76-HAZEL (42935) to request a referral.
